HOUSTON (KHOU) -- Investigators are trying to figure out how a disabled woman was left in a facility van for five hours.

It happened at the Sean Ashley House in the 5600 block of Parkersburg Drive.

Rebekah Rose Harkins, 57, was discovered at 2:30 p.m., dehydrated and unconscious, and rushed to Southwest Memorial Hospital.

Harkins was picked up in the morning from the facility where she lives and driven to the center along with some other clients.

That was at 9:30 a.m.

As the staff manager went out to prepare the vans for afternoon drop-off, Harkins was discovered.

"She was dehydrated when she got to the hospital and unconscious," said Sergeant T. Richardson. "She was not responding. She's at the hospital intubated and she's holding her own right now."

The van driver and other employees are being questioned. At this point, no charges have been filed.

The sign outside the home says it services individuals with autism, blindness, mental retardation and other disabilities.

Harkins is non-verbal autistic and has Turners syndrome.
